SEMINAR REGISTRATION INSTRUCTIONS
PUBLIC SPEAKING ENGAGEMENTS - Upon request, Dr. Smith will bring nutrition and and health related seminars to your area, club and organization, or you may choose any nutritional topic of interest for her to present. Announcements of these events will be posted on the Seminar Announcement web page so that you may attend those engagements open to the public.
SCHEDULE UPDATES - Be sure to check our Seminar Announcement web page for postings of upcoming events. Each summer, Dr. Smith holds a Seminar, generally in July, for clients to share information from scientific symposiums, professional seminars and training, that has not been shared at their appointments. The Public and Family/Friends of Clients are welcomed!
SEMINAR REGISTRATION FORM - Below you will scroll to the Registration Form to complete for when paying to register for our seminars and special events. There is a specific due date for registration, which is generally four weeks prior to the Seminar date. Please answer every questions so we may serve you best. It is important to provide your email and two phone numbers in the event we need to contact you or email some preliminary information. For friends/family, who are not online, they may call to register. If registering at another place of business, other than A.C.N., please be sure that all the information you provide is legible. There is a lot of work to organize a seminar and taking time to recall people because we can't read their writing is a waste of valuable time, so we appreciate your thoughtfulness and carefulness.
PRE-PAID, REGISTRATION IS REQUIRED PRIOR TO THE SEMINAR OR EVENT To insure that each participant receives their Seminar Education Booklet, that we reserve a room with sufficient seating for all will attend and to cover the costs incurred in the preparation of the Seminar, pre-paid, registration is required. In the event you are unable to attend, no worry, you will receive your Seminar Education Booklet, which is the next best thing to being there. No refunds on Seminar Fee.
CAN'T BE IN TWO PLACES AT ONCE - Should a Seminar date fall on the date of a previous commitment, which cannot be changed, simply pay the seminar fee and complete the Registration Form, indicating on the form that you are unable to attend. For those who pay and cannot attend due to a pre-scheduled same date event or an emergency, Dr. Smith provides additional information in their Booklet, when any part of the presentation is specific to those unable to attend, and includes copies of her seminar notes. Again, it is the next best thing to being there.
