common question asked is, "Does Insurance Cover Your Services?"
Answer: Some do and some don't.
Payment for our services are by credit, debit, travel or check cards (Visa, MasterCard or Discover) and clients file their own insurance claims for reimbursement, when covered. Therefore, we have provided the CPT Codes for our services in the PDF Document above. With the CPT Codes, their medical diagnosis(ses) and its ICD-9 Code(s) from their physician(s), clients can check with their insurance company to see what they will reimburse. Also, clients receive copies of all their Lab Reports so they may submit them to insurance companies and physicians when needed. To further assist our clients, the Laboratory Reports for blood, saliva, urine and stool, also, includes the name of a physician.
Visitors to our website, who are interested in becoming a client, may also use these guidelines to check with their insurance company prior to payment for their Initial Assessment, in case services are covered.
A Clinical Nutrition Analysis of Laboratory Reports is provided to identify nutrient deficiencies, biochemical imbalances and organ/gland dysfunctions, not for the diagnosis of any medical condition, disorder or disease. Therefore, providing a Medical Diagnosis for the therapeutic supplement and dietary programs designed by Dr. Smith, based on abnormal nutritional findings, is outside the scope of a clinical nutrition practice. However, it is within the scope of a clinical nutrition practice to provide a therapeutic supplement and dietary support program for a client's medical diagnosis received through their physician.
After becoming a client, should their insurance company request a letter regarding clinical nutrition services, the client may compose a draft of the letter and submit it to Dr. Smith. If the information requested is within the scope of a clinical nutrition practice, Dr. Smith will be glad to finalize the letter and mail it to the client, or if the client provides the address and company name, we will be glad to mail it directly to the insurance company, including mailing a copy to the client for their records, whichever is preferred.
